Oh, maybe an alert would be sent to the person who got the female pregnant whenever she was being attacked?

"The mother of your child is being attacked."

Maybe every once in awhile the mother can take 1 damage every time the baby kicks

Maybe a form of astral insanity; to emulate the mood swings. And you get hungrier faster.

Towards the end of the pregnancy - the last three or four IC months at least, women should have some sort of walking impediment. Maybe walking from room to room should take balance or something and when they walk, everyone would see "(Preggo's name) waddles into the room." and "(Preggo's name) waddles out of the room, clutching her back." And they should lose access to any sprinting type abilities they might have.

They need to eat more often.
