Its simple, using ghost casts you from your mortal body to become a spirit. As a spirit, you can't do anything physical like touch or attack someone. When you return to your mortal body, the physical defense should be stripped away the point you turned ghost.

When you tumble, there's a chance you lose some phyiscal defenses.

Just like any physical affliction shouldn't affect ghost and when you return to your mortal body, the physical afflictions should be stripped away.
And, when you turn ghost, you should drop everything from your inventory, pack, all your jewelry and clothes.
