I can't say I've ever in my life bothered to use Secrets. If it's important, I'll find out from someone else or a map. Maybe have the Specs simply show all exits on QL and such, without having to bother with Secrets at all?

I've found secrets to be so useful, that I trigger it to highlight when I use it as a sign that I might have stupidity and accidently did it. That's how awesome I've found it to be.

It's useful if you have the patience. Though many hidden exits are hidden to a point where you won't notice them even with secrets. In one Imperian skillset, though, there was a wonderful skill that allowed you to see secrets-hidden exits. I've gone walking one day after I learned it and gone OMG, what are those (hidden) ex... OH MY GODS!

Agreed. A long time ago, I needed to find a hidden room, so I just walked along the path trying to walk into every single wall till I found it. Thankfully, I found it after the first dozen rooms or so.

I remember in achaea I was following this orphan around, and he wandered into a hidden room. I found at least 3 hidden exits there from various things leading me there.

I think what it does is fine. SECRETS takes equilibrium, and requires you to suspect there are hidden exits. Concealed exits still have a chance of serving their purpose, just a far lesser one.
