I'm pretty sure you hit faster than regen+mending cure.
AND, if you get it on the first hit, the second hit to the head could give the instakill. Kinda crazy.
THAT, and if you don't get lucky and get first hit->part 1, second hit -> instakill, and they MANAGE to pull off the cure they could still be off-health balance and your next combo to the head does what I first said: left arm hit gives aff, second arm hit gives kill.

BUT it would make bashbrains a damn sigh harder to get, with the inaccuracy of smite down and the chance to actually bashbrain being relatively low also would make it a nerf to the skill.
Yes you can get lucky with a double smite down, but then you can't knockdown smitedown straight away.
If you actually let me get smite down to your head a few times in a row to get this off, then you either suck or you have both legs mangled.
