Fallout 2, and probably the original Fallout as well, might have something you're looking for. I'm not sure to what extent it affects the overall storyline (although I know it does change things) but anything that is alive can be killed. Shopkeepers, pets, small children, and old women are all vulnerable to you, should you choose to kill them. Plus the second one at least is worth playing anyway.
Fallout 2 is my favorite PC RPG of all time...if you're looking to play a bad guy (or lady!) opportunities certainly abound there. You can be a pimp, slaver, mafia enforcer, drug runner, assassin, death cult leader, moonshiner...hell, you can even be a porn star at the Golden Globes if that's your thing...not that that is particularly villainous, but hey!...The best part of it all is a perfect blend of gritty realism and insane, twisted humor.
Of course, you could also play a saint, if you prefer. The beauty of Fallout 2 is an incredibly dynamic storyline that reacts to your actions and the inherent qualities you create your character with.
And did I mention there are rocket-propelled sledgehammers? :D
