IRE Requires some level of suspension of disbelief. You cannot expect to be fully submersed in character and be in that character's place for everything. Well, you can, but for a series of MUDs in which the context is built like IRE's are, you'll have a damn hard time of it.
Saying 15k is generally acceptable. Sure, you'd rather hear fifteen thousand, but whatever.
Saying I have to def up privately is generally acceptable, if you have to say it in public, let me get my defenses. You don't say 'I MUST PREPARE TO DEFEND MYSELF, LOL'. The things you can do to buff your character are widely known through the world, and I would say as an IC thing, as defenses. It's a mechanic of the world in which you play. You don't say 'USE YOUR FIRE OF COSMIC ENERGY ON THAT ASTRAL CREATURE!', you say 'Cosmicfire that lobstrosity.' It's sort of the same thing.
Bashing still pisses me off. You're going to go hunt creatures, you're going hunting.
Levels is a definate no-no, they're circles of experience, and talking about them is generally accepted.
Talk about damage in tells if you have to, I wouldn't be so overboard as to say 'You greatly wounded me!' when someone asks you in a tell how much damage you did.

Here are some rules that are broken a lot.. the ones I'm mentioning are the ones that actually bother me.
1) Testing skills at the Nexi
2) Asking how much damage they do when they hit them with such and such attack
3) Random afflicting.. anywhere, without warning
4) When people say, "deffing up" and then spam the shit out of everyone.
5) Spam of any kind.. emotes.. says.. whatever.
Sometimes I slip and do some of these.. like saying comms instead of commodities and GR/CR instead of guild/city rank.. stupid stuff like that. It's hard to be completely IC when talking to novices and that's understandable, but you should make it known that you're going OOC when you're teaching them or else you're just training them to be OOC as well.

Deffing up and Rezz, I agree with.
12hp however I do not. I as a player, have NO clue what losing 12 hp means, or losing 1000 hp means. I don't know if its a cut a scratch, a lessening of my will to live, a level of Pain... no clue.
So if someone started talkin to me and NOT using the hp numbers then I'll have to send theim private tells saying "HUH? whats that mean?
However, the chrachters know exactly what losing 12hp means.. they feel it, they can explain it to eachother. when a player types 12hp, it just means that the charachter is saying something that to mean,means 12 hp;

well resurrection is a skill, shortening a word can be IC, same for deffing up, which is short for 'I'm putting on my defensive spells'
I really don't see how you can all say oh that is not IC, look at RL military
terms like 'DEFCON, SITREP, CO, XO, SATCOM, RECON, REKI,...' are all shortened versions of other words that later became official abbreviations.
So a military organisation in Lusternia could easily make use of abbreviations for fast ordering like 'rezz, deffing'
There is however, no excuse for 12hp

To this list I would like to add: ratting at the nexus, singing bard songs on autoplay at the nexus (Mitch), being an ass at the nexus, being overtly disgustingly snuggly at the nexus, people bragging despite the listener clearly not being interested yet trying not to be rude, and... more.
