When I was in college I took a graphics class where the final assignment was to design a first person shooter, where we had to work in groups of three.
The day we got the assignment I created an svn repository and sent emails to my two group members. One of them was a pretty good friend and immediately got set up and started helping me out with it. The other guy, who we didn't know, ended up in our group because we were the only one with two people. I had never even saw him in class before, which was a bad sign.
After a week of no response, I sent the prof an email, who told us he would try to get in touch with him. In the mean time he said to only work on 2/3 of the project, and that's all we'd be graded on.
That guy didn't respond to my first email until over a month into the project, THE DAY BEFORE before the deadline. At this point I had put an obscene number of hours into it, probably around 50 hours.
It was pretty awesome being able to respond to him, my email was basically "Sorry, you aren't in our group any more. Good luck finishing this by tomorrow."
Guy didn't show up to the presentation day.

The very worst teamwork experience I've ever had in school goes as such:
When I was in college, I took this advertising basics class and at the end of the semester, we had to pair up and do this project where we pitched an advertising campaign complete with budgets and artwork and schedules and everything for three major media types (tv, radio, print). I got partnered up with this guy and we split up the work. I was the better artist, so I was in charge of drawing up all the storyboards and print ads and what not. We collaborated on the script for the tv and radio commercials and copy for the print ads. He was in charge of getting all the financial stuff together, how much it would cost to do the spots, run the ads, etc and create our budget (we had a limited amount of money we were allowed to spend).
So, a few days before the presentation, all my stuff is done. All the stuff we worked on together is done. The only problem is the stuff he was specifically in charge of. And I couldn't get ahold of him. He wasn't coming to class, he wasn't answering my calls or emails, nothing. He just dropped off the face of the earth. For all I knew the guy had died or something. So, now I had to scramble to try to get all that information he was supposed to get compile it all together and come up with figures for each campaign. And I suck at math. Plus, some of the stuff I just couldn't get cause it was the weekend and I just kept hoping that he'd show up on presentation day and have it all and then I could wait till after the presentation that counted for a third of our grade to murder him to death.
He didn't show up, of course. So, I did the presentation by myself and had to just make something up for all the TV stuff (I got the info I needed for the print and radio stuff). The people I was presenting to called me on the BS budgeting for the TV stuff, but liked everything else and I got an B on the presentation.
I actually saw the guy later that week in the cafeteria and had to leave the room before I stabbed him in the eye with my fork. Apparently he decided to drop the class rather than have to finish that project.

I know, right? I'm still bitter about it, years later. There's like two things ever that made me so mad that thinking about them years later ticks me off, and that was one of them.
Not even so much for the B I got, cause a B isn't a bad grade and I'd done well enough on the rest of the semester's stuff that I got an A on the class anyway, but it was just the inconsideration that made me so freakin mad!

See in that situation, get out early, not when you're going to screw over people. I've only had to do that once, and it was because I spent an atrocious amount of time doing a rough draft of an essay for a group project (in a class that was taken mostly for fun) and had precious few time to devote to studying for a test due that same day (which was not taken for fun). The fact it was a group project made me hesitate to dump it immediately, so I did my work, failed my test, and then bailed the extra class. As for the next test, I studied and did decently on it!
Lesson learned: never take a 400 level sociology class for fun.
