Cavalier's been in play for a little while and I've opted to make a few
small changes in response.

1) Skewer can now be performed with only one of the two pre-requisite
afflictions, but requires both chest and gut to be in critical
condition.

2) Cavaliers can now rend their impales.

3) FractureSkull is now a medium wound affliction.

We have also noted a tricky situation where some people have achieved
100% movement resistance. We did not intend for this to pe possible, so
have now added diminishing returns to movement resistance over 50%, with
a hard cap at 90% movement resistance. If STAT shows you as having
"unearthly" resistance, you have hit the hard cap and will not be able
to go further, even with artifacts.
