Monk Changes

Some changes around monks:
+ The first kata form will occur faster than it had before, but
subsequent forms will be a little slower.
+ Kata terminate's balance loss has been greatly reduced.
+ Break and stun cost more ka.
+ Punch does less wounding.
+ Razing modifiers (skive et al) cost more ka, but they do not slow the
attack at all. The attack will go the same speed as without the
modifier.
+ Added new stand-alone razing skill in Kata. It's primarily meant for
use outside Kata forms.
+ There is messaging when you get a rupture for the victim and the
attacker.
+ Healing scrolls cure a random rupture.
+ It is much more difficult to disrupt a kata form from completing now.
A few examples... If stunned, the form will wait until both balance and
stun are recovered and then continue to the next form. If an arm is
broken, that arm's attack will fail but all the rest in the form will
fire and the remaining forms will be carried out. Etc.
