Mortal reviewers are either slacking or gone, or everyone is designing SO much that nobody can keep up with checking the designs. And when they get checked and returned instead of approved, and often they are returned with invalid requests for adjustments, they then sit in returned queue for two weeks when new designs are being approved right and left. What is going on?

Indeed.
I've had the same designs in submission for three weeks now. Another was rejected, telling me to switch a 'has' for 'have'. I did so and resubmitted it, only to have it rejected, asking me to change the same 'have' back to 'has'. The most frustrating thing is that in between each step is more than a week of waiting.
It's a bit annoying, though I can certainly understand that they're swamped.
And I'm waiting on commodity additions that I'd like as soon as possible...

The more times it has been rejected, the fewer people are able to review it, so the longer it takes to go through a second time. (Plus the more active reviewers have already has their chance, so now it's less active ones who can see it.) If you dispute the reasons for the rejection, it goes to the Charites for adjudication, not the mortal reviewers, which makes it take even longer.

Precisely, what bothers me most is that if everything is fine, the design gets accepted right away. If there's something to correct though, it will sit in queue for weeks when new designs are being approved right away. And the worst is when the requests for adjustments are clearly wrong and through no fault of our own we are waiting for weeks.

That's true but I'm more concerned with invalid requests for fixes (such as Sotsepmet's example) which force me to wait weeks for my designs.

I've heard the reason for this is because designs require a certain threshold of approval or rejection before a decision is made, but people who originally reject do not get to review it on resubmission, and so it sits in stasis with neither enough support to approve or reject. I found that recalling/resubmitting fixed this the last time that I had the issue. I assume that this works because it clears off who has originally reviewed a design, and so the self-check system where reviewers who make mistakes are held accountable fails... and then they just... keep making mistakes. But at least we get our designs!

No, recalling and resubmitting does not clear the list of who's rejected it before (and thus can't rereview it). All it does is clear any pending reviews, so it goes back to the beginning of where it was last time it was submitted, and makes the reviewers who already reviewed it this time through have to do it again.
